Dear Bayview on the Lake Condominium Owners Association,

I understand that according to the Bayview on the Lake Condominium covenants and rules, I must submit a description of my unit remodel and have it approved by the condominium board of directors. The Bayview on the Lake Condominium Association Board of Directors must approve my remodel plans and scope of work before construction can begin. I understand that this review can take up to 30 days.

Therefore, I submit this form with attached plans containing my remodel description and information. Below are some of the common Bayview on the Lake Condominium Association remodeling understandings and conditions, all of which apply to my proposed remodel, unless specific permission has been granted by the board through the approval of this application:
1.The applicable Building Codes for the relocation of any electrical and/or plumbing items will be enforced via any permits required by the City of Kirkland.
2.Water heater replacement will be done by installers credentialed to do this, and by code.
3.Common area utility easements (venting, plumbing, fireplaces, etc.) will remain as is.
4.Concrete walls, floors, ceilings, and exterior walls will not be penetrated. All attachments to the concrete walls, floor, and ceilings must be shown on the plans and approved.
5.All Bayview rules will be observed during construction.
6.Construction noise will occur between 8 a.m. and 5 p.m. only.
7.Construction parking will use the space numbers indicated in this submitted form.  Other parking spaces used must only be used with permission, as these parking spaces are private property.  
8.Any messes that occur in the common areas (parking lot, elevator, walkways, corridors, stairs, or unit entrance areas) will be cleaned up promptly.
9.The areas mentioned in #8 will be left clean and tidy at the end of the workday, 5 p.m.
10.Construction materials and equipment will not be left outside the unit.
11.If the lanai or your garage is used for sawing, dust will be contained by appropriate means, and materials will not be allowed to blow beyond your entry, patio, lanai or your garage space.
12.The elevator will be protected and left safe and tidy each day. All corrugated paper, tape, etc., shall be cleaned up by 5 p.m. each Friday.
13.Water, electrical, or elevator shut off or restriction notices should be pre-arranged with your building’s designated board member. Approved notices should be posted on each affected unit door using only yellow, green or blue easy release masking tape, and on the mailbox bulletin board at least 48 hours in advance, unless it is an immediate emergency.
14. All flooring changes must have acoustic underlayment to lessen the effects of noise on adjacent units. A IIC rating of 50 or higher is required. In general, this can be accomplished with the installation of a 5mm or thicker acoustic underlayment installed under ANY flooring system or flooring material in any part of the home. Provide details and specifications with your flooring submittal.  The Association will utilize this chart to determine whether the appropriate level of acoustic membrane is being provided: IIC Rating Calculator & IIC Floor Ratings Chart - Commercial Acoustics (commercial-acoustics.com).
15.The fireplaces were installed in the 1970’s and have limited heat capacity. Therefore, any fireplace revision must be approved by the Board and permitted by the City of Kirkland Building and/or Fire Departments.
16.Owner assumes full responsibility for any damage that occurs during construction and thereafter, due to construction.  THE BAYVIEW BOARD'S APPROVAL WILL BE CONTINGENT UPON RECEIVING OWNER’S WRITTEN PLANS, SCOPE, AND TIME LINE, ACCEPTANCE OF THOSE PLANS, SCOPE, AND TIME LINE, AND UPON SUBMITTAL OF THIS REMODEL REQUEST FORM WHICH ACKNOWLEDGES AND ACCEPTS THIS RESPONSIBILITY AS A CONDITION OF SUBMITTAL.  
17.As owner, I accept responsibility to have my contractor(s) and workers meet these conditions.
18. These references were included in the resale certificate, which was accepted when the unit was purchased. The CC&R’s, Covenants, Conditions and Restrictions, are the Bayview on the Lake Condominium Association governing documents referenced below:
•Declarations, revised 5-19-2000 and all subsequent amendments thereto, particularly but not limited to pp.30-32 Articles 22 & 23
•Bylaws, amended February, 2001 and all subsequent amendments thereto, particularly but not limited to Article 6, pp. 9 & 10
•Rules and Regulations, revised 7-18-2024, and all subsequent amendments thereto

My submittal of this online form represents my acceptance of these and any other conditions provided by the board absent or with a physical signature.  After Board approval, a letter of conditions of approval will be issued in conformance with the governing documents.  By submitting this form, I assume responsibility for completing the project in accordance with conditions of approval, assume responsibility for any damage due to construction, and I agree to the timelines and rules of the governing documents and as specified in approvals issued by the Association.
