Reimbursement Request Form

Please use this form to submit a reimbursement request to the BOTL Condominium Owners Association for a purchase made on behalf of the association using your personal funds.  Presuming the reimbursement is approved by the association, payment should be sent to you within 3 weeks.

NOTE: using personal funds to purchase items for the Association is not the preferred practice.  The need for this does occur from time to time, and that is the purpose of this reimbursement form.  With little exception, specific authority should be granted from the directors before making a purchase on behalf of the association using personal funds.
