Meeting Proxy

By submitting this form you are allowing the designee to hold your proxy to represent you if you are absent from the meeting.  If the proxy holder is present, this representation counts your Ownership share as being present at the meeting to count toward our quorum.  Additionally, you authorize the proxy holder to cast votes on your behalf. Further, you authorize that your proxy is valid for 11 months from the submittal date, unless the proxy is revoked by you in writing.
